Health chiefs say a culture shift will be needed for their vision to integrate health and social care in the county for it to be a success.
Herefordshire NHS Clinical Commissioning Group leaders presented part of their five year integration plan.
They want he county to be one of healthy individuals living within healthy communities.
To achieve this, their proposals seek to move towards a new system which they say will give patients more options in terms of how they are cared for.
During the first year of their plan, the main focus will be on building community resilience and creating integrated primary, community and mental health services.
They will also focus on improving urgent, dementia and end of life care.
Their second year priorities focus on prevention, psychological interventions and dealing with complex mental health needs. They will also be looking at back office, infrastructure and their estates.
“The plans are intended to move our health and social care system to a new service model in which patients get more options, better support and properly joined up care at the right time in the optimal care setting which will support communities to remain within their own homes and reduce the need for hospitalisation and long term care,” the report presented to Herefordshire Council’s adults and wellbeing scrutiny committee read.
“This will support our objectives of building community resilience and tackling health inequalities.”
The changes form part of the NHS Long Term Plan and health chiefs say that by providing services locally it will support the local council’s mission to allow residents to live safe, healthy and independent lives.
