Baroness Valerie Amos has been appointed to lead a new national investigation into maternity and neonatal care.
Health Secretary Wes Streeting says families wanted “fresh eyes” and independence following scandals such as the Shrewsbury and Telford inquiry, which found hundreds of avoidable deaths and injuries.
The review will examine up to ten services, bringing past reports together into one national set of actions.
