Officers are appealing for witnesses to a collision that happened around 5.30pm on Sunday 22 October, involving a Silver Peugeot 207 estate and a green Rover 75 saloon car.
The Silver Peugeot was travelling along the A438 at Hollybush towards Ledbury when it was involved in a collision with the green Rover travelling in the opposite direction, towards Tewkesbury.
The front seat passenger of the Rover, a woman in her 70s, was taken to Hereford Hospital with internal injuries and sadly later died.
A man in his 70s who was driving the Rover was also taken to Hereford Hospital with chest injuries and is currently receiving treatment.
The driver of the silver Peugeot, a woman in her 50s, was treated by paramedics at the scene for what are believed to be minor injuries.
Police would like to hear from anyone who witnessed the collision or anybody who saw the manner in which either vehicle was being driven beforehand.
